If that index is undefined ... I wonder, do you still have your SETTINGS table ? Or better said, do you have all fields O.o

Lets try to replicate what has happened....
You created a backup of your SQL database?
You created a backup of your PHP file?
You uploaded the new PHP files?
You started the upgrade script?
