Issue #361 and Issue #363

Bug Source: XMB, all versions

Symptoms: File uploads sometimes fail without any error message displayed to the user. Failed uploads sometimes leave orphaned files in the system upload temporary directory. These bugs apply to all web uploads, including forum post attachments and Administration Panel operations.

Security Impact: none

Fixed By: XMB-1.9.11.09.zip and later are not affected.
